  • To notify any Infectious Disease.
  • To comply with Court of Law Request, e.g. subpoena.

Your medical record is a confidential document. It is the policy of this practice to maintain the security of all personal health information at all times, ensuring it is accessible only to authorised staff in compliance with legal and regulatory requirements.

Any patient documents are securely scanned and then disposed of in confidential bins for shredding.

If you have any concerns or wish to place restrictions on access to your personal health information, please discuss this with your doctor.
